"I solemnly swear that I was born at Calumet, Houghton County, in the State of Michigan, on or about the 30th day of August, 1881, that my father Nicholas F. King, was born in Cornwall, England; that he emigrated to the United States from the port of Liverpool, England, on or about Aug. 1, 1867; that he resided 51 years, uninterruptedly, in the United States, from 1867 to 1918, at Calumet, Michigan; that he was naturalized as a citizen of the United States before the Circuit Court in Houghton County, at Houghton, Michigan (claiming citizenship through his father's naturalization); that I am domiciled in the United States, my permanent residence being at La Grande in the State of Oregon, where I follow the occupation of General Secretary Y.M.C.A., and that I am about to go abroad temporarily; and I intend to return to the United States within one [year?] with the purpose of residing and performing the duties of citizenship therein; and that I desire passport for use in visiting the countries hereinafter named for the following purpose:
London England and Paris France For Y.M.C.A. service with the troops of the American Expeditionary force.
I intend to leave the United States from the port of New York, [traveling] on board the French Line Steamer (name unknown) on June 27, 1918.
